Valsalva通気法の練習装置の考案

A New Device for Training of Valsalva Maneuver

I．はじめに

　耳管通気法にはいくつかの種類があるが，その中でValsalva通気法は特別な器具を使わなくても患者が自分自身でできる簡便な通気法である。しかし耳管機能が正常な者でも通気のコツがのみ込めないためにうまく通気ができない者が少なからずいる。ことに幼少児の場合には通気のやり方を教えても説明が十分理解できず，Valsalva通気法を行わせることができない場合が多い。またなんとか方法を理解させたとしても，患児が本当に鼻咽腔内圧を十分に高めて通気を行っているか疑わしいことが大部分である。そのため自分で耳管通気ができるというValsalva通気法の長所を活用できず，特殊な器具（Politzer球，通気カテーテル）を用いて通院により通気を行わざるをえなくなる。一方日常外来診療で繰り返し通気を行う必要のある症例は多く，Valsalva通気法を能率よく患者に教える必要が痛感された。そこで簡単な通気の練習装置を作成し実際に患者に使用させてみた。この装置は容易かつ確実にValsalva通気法を患者に教えるという点では非常に有効で，繰返しのValsalva通気でよい結果を得た症例がかなりあるので，この装置の紹介とこれを実際に用いてみた経験について報告する。

　A simple device for training of Valsalva maneuver in patients with the middle ear deseases is presented. This device consists of a vinyl resin tube connected with an olive-shaped nozzle of Politzer's bag and a water container. The patient is asked to apply the nozzle to his naris and expire as much as he can produce air bubbles in water. Then he will be able to do Valsalva's maneuver without the device successfully. Trying to produce air bubbles, patients will easily learn how to elevate the nasopharyngeal air pressure even in infantile patients.
