側頭骨のCT診断—内耳奇形

Computed Tomography of Congenital Inner Ear Anomalies

I．緒言

　先天性難聴の診断には聴力，前庭機能検査などと共に，X線検査が重要な意義をもつ。この場合，難聴は奇形に起因することがあるが，伝音奇形についてはX線学的な情報が得られやすく，伝音系再建術の適用，あるいは手術法の選択に有用な情報を提供する。他方，内耳奇形には骨迷路と膜迷路の障害がみられるが，このうちX線学的診断の対象となるのは骨迷路の異常であって，その適用はかなり限定されると言ってよい。しかし，奇形耳では検査対象が低年齢層に多いこともあって，聴力，前庭検査などで十分な情報が得られないことがある。したがって，その異常を画像として把握できれば，診断を補完，ないし確定できる利点がある。

　そこで，高度難聴症例に対する検査の一環としてCT検査を施行した。そのなかに内耳奇形が認められ，その診断にCTが有用であったので報告する。

　Computed tomographic studies were carried out on patients with congenital deafness. In this series, inner ear anomalies were demonstrated in 6 ears of 4 cases, the anomalies being bilateral in 2 cases.

　In three ears from two patients, aplasia of all inner ear structures was found. On the right ear from another patient, there were anomalies of the semicircular canals associated with partial absence of the middle turns of the cochlea with presence of the basal turn. On the left ear, anomalies of the semicircular canals and vestibule were demonstrated. In the remaining patient, anomaly was only characterized by partial osseous obliteration of the basal turn of cochlea.

　CT is the examination of choice for demonstrating inner ear anomaly.
