長期間の呼吸管理を必要とした急性喉頭気管気管支炎の1例

Acute Laryngotracheobronchitis in Adult: Report of A Case

I．はじめに

　急性喉頭気管気管支炎（acute laryngotrachcobronchitis）は主として幼小児にみられ，進行性の呼吸困難をきたし，適切な治療がほどこされないと重篤な結果になる疾患である。本症はこれまでにも多数の報告があるが，成人では稀であり，また実際に内視鏡写真が示されているものはほとんどない。今回我々は成人で，この疾患のため長時間の呼吸管理を必要とした症例に，経過観察のため数回，気管支ファイバースコピーを施行する機会を得たので，若干の文献的考察を加えて報告する。

　Acute laryngotracheobronchitis is commonly seen in infants and younger children, but rare in adults. In a 57-year-old female with acute laryngotrachcobronchitis accompanied with serious dyspnca, tracheostomy and energetic respiratory assistances were performed. The fiberscopic views of her tracheobronchial mucosa showed strong edema and inflammation. In laboratory data, parainfluenza 3 virus was identified. Steroids and antibiotics were administrated, and her clinical course of the following one month was presented. Bibliographical review was also made.
