NLA変法麻酔の鼻副鼻腔手術への応用

Modified NLA Applied to the Sinus Surgery

I．緒言

　従来から鼻副鼻腔領域における良性疾患の手術は局所麻酔で行われることが一般的である。

　しかし，局所麻酔剤ではしばしばショックが起こり得ること，咽頭流血により患者に不快感をあたえること，上顎洞膜様部，前部篩骨洞の麻酔は十分に行い得ないことなどの点を考えると，患者にとっても術者にとっても全身麻酔の方が手術を行いやすいと考えられる。とくに近年の全身麻酔法の進歩はめざましく，従って患者側から全身麻酔による手術を希望されることが多くなりつつあるのが現状である。

　Fifty-six patients underwent sinus surgery under modified neuroleptanesthesia with fentanyl, diazepam, nitrous oxide and oxygen.

　The results of this method were summerized as follows ;

　1) The ammount of blood loss in this method with fentanyl was not so remarkably greater than that with pentazocin and that with local anesthesia.

　2) Patients were very rapidly recovered from anesthesia and were able to respond the verbal commands soon after extubation.

　3) Postoperative pain was mild, and vomiting was seldom recognized.

　These advantages suggest that this method is one of the most preferable anesthesia for sinus surgery.
