I．緒言

　喉頭の粘膜上皮は，部位により性状が異なる。喉頭蓋の舌根面および喉頭蓋の上半部，声帯ひだの一部，室ひだの一部は重層扁平上皮で，その他の部分は多列線毛上皮である。この線毛上皮問には，杯細胞が散在し粘液を分泌しており，また，上皮には粘漿液腺である付属小腺（喉頭腺）が開き，特に喉頭蓋の喉頭面，室ひだ，横披裂筋の間によく発達している^1）。このため，喉頭における腺癌は，他の頭頸部領域の悪性腫瘍に比し多くあるべきである。しかるに，現実には，そのほとんどは扁平上皮癌であり，喉頭腺癌の発生率は欧米では，Toomey^2）の全喉頭癌の0.5〜0.9％，我国では，国立がんセンター^5）の0.7％，佐藤ら^4）の0.3％などと，極めて稀である。

　最近，我々は2例の喉頭腺癌を経験したので，臨床的，組織病理学的に検討を加え，ここに報告する。

　Two cases of the laryngeal adenocarcinoma were reported.

　Case 1 ; a 57-year-old woman had adenoid cystic carcinoma with supraglottic type (T₂N₁M₀) and treated by a total laryngectomy with the left radical neck dissection.

　Case 2 ; a 65-year-old man had adenocarcinoma with supraglottic type (T₁N₁M₀) and treated by irradiation (total dose : 6000 rads) and the right radical neck dissection. No evidences of the tumor recurrence or metastasis in both cases were confirmed up to day. Both cases were histologically confirmed that the tumor was originated in the laryngeal glands. Irradiation therapy for laryngeal adenocarcinoma was discussed with respect to the therapeutic effectiveness. Our second case suggested good effects of irradiation therapy.
