小児横紋筋肉腫二症例

RHABDOMYOSARCOMA IN CHILDREN, A REPORT OF Two CASES

　I．緒　言　横紋筋肉腫は比較的稀な疾患とされており，またきわめて予後の不良なものとして知られている。しかしながら病理組組学的診断基準の確立により本症の報告は年々増加している。頭頸部領域は本症の好発部の1つであり，われわれ耳鼻咽喉科医はこれに遭遇する機会が多いと考えられる。一方，1961年PinkelおよびPickren^1）が横紋筋肉腫に対し，手術，放射線治療および化学療法を組合せる，いわゆる三者併用療法を行なつて以来，特に小児横紋筋肉腫の治療成績は著明に向上している。したがつて現在では治癒可能な悪性腫瘍の1つとしてその認識が改たまりつつある。

　われわれは最近6歳男児の耳下腺周囲軟部組織原発と考えられる胞巣型横紋筋肉腫と2歳男児の鼻腔原発と考えられる胎児型横紋筋肉腫症例に対し，Sutow^2）の提唱したいわゆるVAC療法を行ない，現在も鋭意加療中であるが，良好な経過を辿つているのでここに報告する。

　Two cases of childhood rhabdomyosarcoma are reported.

　Case 1, a boy, aged 6, complaining of swelling at the right subauricular region, was diagnosed by biopsy as having alveolar rhabdomyosarcoma. Radiation therapy and so-called VAC therapy proposed by Sutow in 1969, were performed and the tumor disappeared rapidly, and the patient has been well for eleven months.

　Case 2, a boy, aged 2, presented a tumor in the right nasal cavity. The tumor was removed under general anesthesia and was diagnosed as embryonal rhabdomyosarcoma. Radiation therapy and VAC therapy were performed. There has been no evidence of local recurrence for nine months.

　We experienced 5 cases of childhood rhabdomyosarcoma including these two cases in fifteen years from 1963 to 1977.
