耳下腺にみられた線維素性唾液管炎の1症例

Sialodochitis Fibrinosa of the Parotid Gland

　I．はじめに

　線維素性唾液管炎（sialodochitis fibrinosa）は1879年にKussmaul^1）により初めて報告された疾患である。本症は線維素性塊状物による唾液管の閉塞で生じる発作性再発性唾液腺腫脹を特徴とするが，病因としてアレルギーが疑われ，アレルギー性耳下腺炎とも呼ばれている^2〜4）。本症は比較的稀な疾患で，本邦では奥田ら^5），宮崎ら^6）の報告をみるにすぎない。

　今回，鼻アレルギーを合併し，両側耳下腺の発作性再発性腫脹をきたした線維素性唾液管炎の1例を経験したので，その臨床症状，検査成績，治療成績を報告し，若干の考察を加え読者諸氏の参考に供したい。

　A 39-year-old female with sialodochitis fibrinosa of the bilateral parotid glands was reported. She suffered allergic rhinitis and showed hypersensitivity to iodine. Swelling of the parotid glands was occurred after frequent attacks of allergic rhinitis or taking foods. Numerous eosinophils were found in mucous plugs extruded from the Stensen's ducts. The eosinophilia and the elevation of serum IgE together with above allergic episode evidenced allergic origin of the disease, but no specific antigen was detected. The repeated injections of methylprednisolone into the parotid glands through the Stensen's ducts after washing the glands controlled subsequent swelling of the parotid glands.
