面側耳後部に発生した軟部好酸球肉芽腫の1例

A Case of Eosinophilic Granuloma in Bilateral Retroauricular Regions

　I．はじめに

　軟部好酸球肉芽腫は慢性に経過し，臨床的には良性の比較的稀な疾患である。全身の軟部組織，特に耳下腺部，顎下腺部，涙腺などの顔面頸部に好発し，単発ないし多発性に腫瘤を形成し，末梢血中の好酸球増加が特徴的である。その特異な病理組織像と臨床所見から，骨，肺，皮膚，消化管，泌尿生殖器などに発生する好酸球肉芽腫とは区別される独立した疾患と考えられている。今なお原因と本態は不明のところが多く，種々の治療法が試みられている。最近われわれは，両側耳後部に対称性に原発した本症の1例を経験したので，その臨床経過を報告し，併せて若干の文献的考察を加える。

A 31-year-old male had bilateral retroauricular tumors, and his blood examination revealed slight increase of eosinophils. Serum IgE was also slightly higher than normal. The tumors were removed surgically and histological examination showed eosinophilic granuloma in the soft tissue. Further-more, mastoidotomy was done to clarify a punched out lesion in skull x-ray in the right side and a spherically developed air cell was found.

After the surgical excision of the tumors, the patient had been treated with Linac irradiation of 2800 rads and of the total dose of 325 mg of pred-nisolone.

There was no recurrence in 2 years after opera-tion.
