上顎洞に発生したspindle cell squamous carcimomaの1例

A Case of Spindle Cell Squamous Carcinoma of the Maxillary Sinus

　I．はじめに

　上顎洞における悪性腫瘍はcarcinomaが最も多く，次いでsarcomaがみられるが，carcinomaの様な悪性上皮成分とsarcoma様の悪性間質成分が同時に同一器管に存在するものは極めて稀である。この種の悪性腫瘍は従来よりcarcinosarcoma（Saphirら^1）），pseudosarcoma（Lane^2）），squamous cell carcinoma with sarcomalike stroma（Appleman^3）），spindle cell squamous carcinoma（Lichtiger^4））等多くの名称が与えられているが，その概念，higstogenesisに関して多くの議論が存在する。耳鼻咽喉科領域では喉頭に発生したものはしばしば報告されているが，上顎洞に発生したものは数例をみるのみである。我々は最近上顎洞に発生したspindlc cell squamous carcinomaを経験したので，その組織学的特徴に文献的考察を加え報告する。

　Malignant neoplasms, showing simultaneous presence of epithelial and nonepithelial elements are rare and these tumors are called as true carcinosarcoma, pseudeosarcoma or spindle cell squamous carcinoma according to the origins of sarcomatous component.

　This report describes the histological characteristics of spindle cell squamous carcinoma of the maxillary sinus as follows: 1) This tumor was mainly composed of anaplastic spindle cells. 2) Some epithelial cell clusters were observed at the peripheral portions. 3) Transitions from squamous to spindle-shaped cells were observed.

　These findings suggest that spindle cells of this tumor are epithelial in ther origin, and they reveals their own malignant potential.
