I．はじめに
頭蓋底に位置する蝶形骨洞および下垂体の病変は，外科的には脳神経外科，耳鼻咽喉科両科の境界領域であり，両科が協力して患者の治療にあたることが少なくない。1976〜1980年の5年間に，東京医科歯科大学脳神経外科教室，昭和大学藤が丘病院脳神経外科教室，武蔵野赤十字病院脳神経外科および東京医科歯科大学耳鼻咽喉科教室が互いに協力して，23例の蝶形骨洞・下垂体手術を施行した。蝶形骨洞・下垂体へのアプローチとしては，鼻中隔経由にて行うKern-Laws法を用いた。本報告ではこの手術法の術式の概略と，われわれの経験した術後合併症について報告する。
Transseptal approach to the sphenoid sinus and the pituitary gland was performed on 23 cases during 1976-1980. Kern-Laws technique was used for this approach. The characteristic feature of this technique was to preserve the anterior part of the septal cartilage. 23 cases included 16 adenomas, one hyperplasia, one ganglioglioma, one chordoma of the sphenoid sinus, one sphenoidal mucocele and 2 histologically unknown cases. As surgical complications, one case of CSF leakage, one hyposmia and one epistaxis were seen.
