　I．はじめに

　気管支異物症の報告はわが国において数多くなされているが，気管支直達鏡下に摘出不能であり開胸術を要したという報告は決して多くはない。われわれは最近，直達鏡下に摘出が困難であり，かつ上行大動脈瘤の存在が強く疑われたため，摘出時外力による危険性を考慮し直達鏡下摘出を断念ぜざるを得なかつた症例を経験したので報告する。

　A 53-year-old male was incidentally found a staple in the right bronchus on a chest film taken for examination of the hypertension. The patient.however, had been well without any subjective symptoms. Bronchoscopic removal was extremely difficult because of granulation tissue surrounding a tip of the staple in the bronchial lumen, and the existence of aortic aneurysms. The staple was removed with bronchotomy 167 days after aspiration. Metallic foreign bodies are considered to be less irritable and tend to be long term foreign bodies if they have appropriate shape and size. The indication of bronchotomy was discussed.
