鼓膜形成におけるoverlay法とunderlay法

OVERLAY AND UNDERLAY METHODS IN TYMPANIC MEMBRANE RECONSTRUCTION

　I．緒言

　鼓膜緊張部の大半が欠損し，かろうじて鼓膜輪のみが健全であるという大穿孔例に対し，その閉鎖手術が問題となるのは，新鼓膜の形成にあたり，どうしても欠かすことのできない移植床をどのように作成するかということである。換言すれば，鼓膜輪とそれに続く周囲組織に本手術成否の鍵が握られているといえよう。すなわちそれをうまく利用することで手術は成功する。この際とりわけ重要な役割を演ずるのは，前および下方の鼓膜輪とそれに隣接する組織で，この処置と利用法が最大の問題であろう。したがつて中等度以下の穿孔であつても，残存鼓膜に著しい萎縮や石灰沈着などがみられ，それが完全に取り除かれないと新鼓膜が作れないという症例，あるいは鼓膜の前端部にのみ限局した辺縁性の穿孔があるという症例では，やはり大穿孔例に対すると同じような処置が必要となるであろう。

　ところで鼓膜の形成法には，大別してoverlay^2）（lateral^5）6），onlay^9））法とunderlay（medial，inlay，interior^10），undersurface^7））法とがある。これは移植片を実際に鼓膜代用として穿孔部に置く場合，移植床として残存鼓膜あるいはさらに鼓膜輪などの線維層を利用することになるが，その際その層の内外いずれの面にそれを求めるかによつて，前記いずれかの名称が付けられる。すなわち外側面を利用すれば，overlay法であり，内側面を利用すれば，underlay法である。しかしoverlay法で移植床を鼓膜の周囲に広く求めすぎると，すなわち外耳道前壁をも広く剥離すると，術後anterior bluntingと呼ばれる鼓膜の膨隆がその前端部，すなわち外耳道前壁移行部で生じ易く，その結果，本来前壁に対しては全体として鋭角に移行すべき鼓膜が鈍角となり，その自然な形が失われてしまう。逆に今度は，underlay法を大穿孔例に用いると，もともと移植床が乏しいこともあつて，ちよつとした接着の不備でも，それが後でスリット状の間隙となり，鼓膜前端に小穿孔を作つてしまうことになる。

　Twenty two cases of total tympanic membrane perforation with intact ossicular chain were selected in order to compare results of overlay and underlay grafting methods.

　In 12 overlay cases the fascia was tucked in the slit between the canal wall skin and the annual remnant, and in 10 underlay cases the fascia was tucked in the slit between the mucosa and the annual remnant. The minimum follow-up period was 4 months and the maximum, 2 years.

　Successful hearing restorations were obtained by either method.

　The use of the fascial graft closed the perforation in 21 cases, In 1 of the underlay cases and 4 of the overlay showed a slit-like anterior perforation in the former and persistence ofslight blunting of the anterior sulcus in the latter. Both the overlay and underlay methods proved to be acceptable procedures. However, itis difficult to determine which is the better grafting procedure in cases of total perforation.
