I．はじめに

　耳管の生理機能の低下は，中耳の炎症性疾患の大きな原因と考えられている。従って耳管機能は鼓室形成術の予後を大きく左右すると予想され，ひいては鼓室形成術の適応という問題をも含んでくる。われわれは耳管の圧平衡能を知るために，鼓膜穿孔耳に対して，inflation-deflation test（以下IDテストと略）を施行している。今回は種々の鼓膜穿孔耳について本検査の結果を報告し，その病態および手術の予後との関連などについて検討を加えた。

　The ventilatory function of the Eustachian tube was measured with inflation-deflation test in 187 ears including 17 ears with traumatic perforation, 67 ears with chronic otitis media, 18 ears with cholesteatoma, 3 ears with adhesive otitis media, and 82 ears with secretory otitis media. The changes of middle ear pressure were classified in four different types, patulous, normal, stenotic and obstructive, depending on the residual positive and negative pressures. The results showed that the residual negative pressure was correlated with pressure equilibration function. More than half of ears with chronic otitis media showed poor function. The ears with cholesteatoma, adhesive otitis media and secretory otitis media showed bad function. The results of tympanoplasty did not always correspond with the results of pre-operative inflation-deflation test.
