外耳道に発生した珍しいSweat Gland Carcinomaの2症例

SWEAT GLAND CARCINOMA OF THE EXTERNAL AUDITORY MEATUS, A REPORT OF TWO CASES

　Ⅰ．緒言

　一般に聴器に発生する癌腫は非常に稀なもので，Robinson^1）は212,000例の耳疾患中，耳腫瘍は48例で，そのうち悪性のものは24例であつたと報告し，Bezold^2）は20,000人の患者の中，4人の耳悪性腫瘍をみたと報告している。また，外耳道の悪性腫瘍についてみると，Robinson^1）は上記24例のうちの数例であつたとし，Fraser^3）は外耳道疾患646例中3例に外耳道悪性腫瘍をみたといい，Peele^4）ももつとも稀な疾患の一つと述べている。一方，汗腺癌についてはStout^5）らの12例の中に外耳道に生じたものはなく，また，Miller^6）が1870年から1967年までの間に，自験例5例を含めて文献的に考察した39例を調べてみても，主として腋窩その他の体表皮膚に発生しており，外耳道に生じた汗腺癌は見当らない。しかし，Risch^7）らの統計的観察によると外耳道における原発癌36例の中に汗腺由来と考えられる腺癌が8例含まれている。私どもはこのきわめて珍しい外耳道汗腺癌を2例経験したので報告する。

　Two cases of sweat gland carcinoma, both, arising from the apocrine sweat glands of the external meatus are reported. In the differential diagnosis of the affections of the external auditory meatus findings of sweat gland carcinoma should be kept in mind.

　As these tumors are rather low in radiosensitivity surgical procedures are the choice in their removal. These two cases were treated surgically.

　No evidence of recurrence is seen in these two cases for two years after the operation; the patients are still under observation.

　The authors propose the term, "sweat gland carcinoma" for these affections.
