Ⅰ．緒言
脳幹とは，延髄・橋・中脳を総称するもので，脳神経核の大部分（視神経と嗅神経を除く）はここに蔵されている。ゆえに，脳幹障害の部位診断に際しては，これら脳神経核およびこの部を通過する各種神経伝導路の障害が，重要な目標症候となる。眼運動系もまた，脳幹で複雑なネットワークを形成しており，この部の障害はしばしば眼球運動の異常として捕えられる。
かように脳幹障害の診断において，眼球運動の占める役割は重要であるが，その示す反応はきわめて多彩であり，前庭眼反射に例をとつてみても，反応の解釈は少ずしも容易でない。理論上，半規管から，第8神経を通り前庭神経核に至るafferent pathway，さらに上位の眼球運動に関与する神経伝導路や介在ノイロンの障害は，すべて反応異常に結びつくものであり，部位診断という見地からの反応解釈には，十分なる考慮が必要である。
Two cases of hemiplegia alternans inferior with marked signs of pontine disturbances accompanied by peculiar eye movements are described.
Case 1. A male, aged 46. Both eyes showed oscillating vertical nystagmus and the patient complained of oscillopsia. Adduction was bilaterally affected on attempted gaze to the side. But convergence was preserved. There was incomplete paralysis of horizontal gaze to the left and right. The vestibular nystagmus was lost; optokinetic nystagmus was evoked with difficulty, showing only inversion phenomenon.
Case 2. A male, aged 63. There was complete paralysis of horizontal gaze to the right. On gazing to the left no adduction of the right eye occured, but the left eye was fully abducted, with simultaneous coarse nystagmus towards the left. And the right eye showed clockwise rotary nystagmus. Vestibular and optokinetic nystagmus failed to be elicited towards the right.
The authors believe that these peculiar eye movements are most likely connected to some disturbance occurring in the region of fasciculus longitudinalis medialis and the paramedian zone of the pontine reticular formation.
