問診を中心としたメマイおよび平衡障害

THE DIAGNOSIS OF DIZZINESS AND LOSS OF BALANCE DEPENDENT UPON A GOOD HISTORY TAKING

　I．はじめに

　メマイおよび平衡障害の発現は身体平衡系の故障に依存している。しかしながらこの系は前庭迷路をはじめとして，視覚器，深部知覚器，さらに上位中枢としての脳幹，小脳，大脳にわたる広範囲なものであり，また他方においてこの系の故障をきたしうる疾患がきわめて多様であるなどの理由によつて，その病巣ならびに原因診断は容易でないことが多い。

　このような意味からメマイ・平衡障害の臨床においては，意欲的かつ的をえた問診がことさら重要であると考えられる。本論文ではこの立場に立つて，問診として取り上げられるべき事項およびその診断学的意義などを中心に，私たちの外来における症例を対象として調査してみた。

　The importance of taking a careful history in reaching a proper diagnosis in patients complaining of dizziness and loss of bodily balance is emphasized and studied with evidence of statistical data.

　This history taking covers the past, familial and present histories, including a careful description of the type of dizziness the patient suffers, whether or not it may involve cochlear symptoms or disturbances in the autonomic nervous system.
