A RECENT EXPERIENCE IN MUSCULOPLASTY

　I．はじめに

　耳手術において広い乳突洞創腔を残した場合に，術後に表皮化障害，外耳性耳漏をきたし，いろいろ煩わしい問題を生ずる。この問題を解決するために諸学者によりさまざまな工夫がこらされている。

　当科では，すでに昭和38年から外耳道後壁を保存して乳突部術創を消失させる方法として，内藤により提唱されたmeatotympanoplastyをmodifyして行なつてきた^18）。今回は広い乳突腔を有する再手術例や，骨性外耳道後壁あるいは軟部外耳道の保存例における再手術時に，有茎側頭筋弁による乳突腔の充填と外耳道の再建を行なつた最近の症例について，若干の考察を加えて報告する。

　By means of pediculated flap of the temporal muscle the wide area of the mastoid wound following reoperation is filled in and external meatuses were reconstructed.

　Eventually all cases made a complete healing but in one case the muscular flap became necrotic and attainment of the desired result in the construction of the external meatus was a failure; three cases showed postoperative infection.
