喉頭疾患の治療に用いた酢酸Dexamethasone（Dectan）の使用経験

　副腎皮質ホルモンがReichstein（1934），Kendal（1936）らによつて抽出されていらい，数十種のステロイドが発見または合成されているが，臨床的には，Kendal及びHench（1948）がはじめてリウマチ疾患に使用し，有効であることが判明した。その後，各種疾患に適用されて，治療効果がつぎつぎに報告されているが，一方副作用についてもいろいろと論議されて，適用疾患及び投与量については，特に周到な注意が必要とされている。耳鼻科領域ではBordley（1949）が鼻アレルギーに使用し有効なることを，また，Silverblatt（1958）が喉頭疾患の吸入療法に使用して，その効果が非常に著明であつたことを報告している。ステロイドホルモンは非特異的刺激による反応性炎症にたいし，消炎または抗炎的に作用し，治癒後の線維組織の増殖にたいし抑制的に働くといわれている。われわれは難治な喉頭疾患の治療方法とその効果について，今まで研究をかさねてきたが，今回ステロイドホルモンの一種である酢酸Dexamethasone（Dectanmerk：9α-fluoro-16α-methylpredonisolone acetate）を慢性喉頭炎および喉頭疾患の術後治療に喉頭噴霧吸入療法の薬剤として使用，治療成績について報告する。

　Dexamethasone nebulization for laryngeal diseases.

　Since the introduction of steroid hormone, many reports have been presented on its effects in various field of clinical medicine.

　At the voice and speech clinic of University of Tokyo, a nebulization of dexamethasone acetate was used for chronic laryngitis vocal cord polypi and recurrent nerve palsy. In the latter two, a dexamethasone nebulization combind with a surgical therapy was applied.

　The nebulization of dexamethasone acetate was found to be satisfactoy for the treatment of chronic inflammation, without any unfavourable side effects.
