化膿性中耳炎の耳科用プレデックス液による治療経験

I．緒言

　耳科用プレデックス液は，1cc中にプレドニソロン，アセテート5mgデキストロマイシン3.5mgプロピオン酸ソーダ50mgを含む中耳及び外耳道の炎症性，細菌性疾患に対する治療用点耳液である。我々は之を用いて急性及び慢性化膿性中耳炎の治療を行い，著効を認めたので報告する。

　デキストロマイシンは本邦土壌中より新しく発見された放線菌の一種Streptomyces fradiaeの一株の産生する新抗生物質で，基準名は硫酸フラジオマイシンといい，硫酸ネオマイシンと同一物質である。グラム陽性，陰性菌，ミコバクテリウムに優れた抗菌作用を有し，特にブドウ球菌には強い抗菌力を現わすが，緑膿菌にはやや効力が弱い。特筆すべきは，溶液の安定性が大であることで，その水溶液は室温で少くとも6カ月間，100℃で20分間加熱しても殆ど力価の減退を認めないことである。又，抗菌力は滲出液，血液成分，酵素などによつて減退せず，耐性菌をつくり難く，現在臨床的にはまだ耐性菌の発現が認められない。

Therapeutic effect of predex aural drops composed of prednisolon, dextromycin and sodium propyon was tested by using it in the treatment of 59 ears affected with acute otitis media, 32 ears with chronic otitis media and in 26 ears in which the ear discharge failed to stop after tympanoplasty. Once a day the ear under treatment was filled with 0.25 cc of pre dex aural drops. In acute otitis media the agent was effective in establishing a cure in 86.3% of the total number ; in chronic otitis media, 59.4% but, in those in whom the perforation aperture was small in size, the effetivity was 72.2% ; in postoperative case the effectivity was no more than 53.9%.
