　顔面神経が側頭骨内を複雑に走行しているために，側頭骨内疾患或はその手術的操作によつて麻痺される危険が少くない。この事は臨床的に古くより注意されて現在に於ても変りがない。

　然しながら慢性中耳炎に際してどの程度に顔面神経がその危険に露されているかは明にされていない。従来からの考え方は真珠腫性中耳炎に於いては顔面神経が犯されて臨床的にも麻痺を認めることが少くないし，一般慢性中耳炎に於ては急性増悪時に顔面神経の麻痺を来し，慢性経過を取ることからすれば結核性中耳炎に於ても顔面神経麻痺を起し易いとされている。

Goto found ten cases in which the facial nerve was found to be exposed among 230 cases of radical mastoidectomies performed for treatment of chronic otitis media. Even among the ten, 8 cases did not show evidence of facial palsy: it is only in such cases that bony involvement is advanced that facial canal may be involved in the destruction. In order to avoid injury to the facial nerve during the course of radical mastiod opera-tions the most favorable results may be obtained by using endaural approach, with constant reliance on irrigation of the field of operation with removal of the debris cau-sed by the use of electric burr by means of suction.
