長期間の髄液漏を見た脳下垂体部腫瘍手術例

　緒言

　脳下垂体部腫瘍の手術は，近時脳外科の著しい進歩により専ら開頭術によるを常識とせられ，かつてHirsch氏により一応完成された経鼻中隔法は其の存在意義が忘れられている様であるが，各部の優秀な抗生物質の発達せる今日，鼻内手術の進歩と相俟ち経鼻法の再認識が成されてよい時代に来ていると考えられる。此の点に就いてはHirsch氏が米国に移つて戦後の手術成績を発表するに及び尚其の感が深い。我が国に於いては戦前に数氏により症例報告がなされているが，其の成績は芳しいものではなかつた。戦後に於いては当教室に於いてのみ行われ，戦後より今日迄の10数例の該手術法による成績は開頭法に優るとも劣らないものがある様である。我々は最近経鼻手術を受けた巨大なParasellar Tumorの患者で，腫瘍の摘出は出来なかつたが，術後相当の長期間に亘り髄液の持続漏出を見，此れがかえつて病状に好影響をもたらし，一時は諸症状の改善を見たが，遂には不幸の転帰をとるに至つた1例に遭遇したので報告する。

　Uchida and Onmyoji report a case of pituitary tumor that caused visionary disturcances in the beginning but finally, the loss of bodily movements due to increased intracranial pressure. The tumor which was large in size was removed incompletely by modified Hirsch's method of approach through the nasal septum. Following this surgical intervention improvement of vision to a certain extent was seen but, cerebral fluid leakage continued without abatement for the period of 6 months. After which the leakage stopped with recurrence of former symptoms and with reestablishment of that leakage abatement was seen in the symptoms. Various means were tried to curtail formation of cerebral fluid without success and the patient finally died.
