硝酸銀結晶による耳瘻管の化膿時の全剔出法

　緒言

　耳瘻管は先天性で，耳輪前方，耳輪脚及び耳輪線上に存在する。その内，耳輪前方及び耳輪脚部にあるものは炎症を起すことがある。この耳瘻管が一度炎症を起すと炎症を繰返しやすくなり瘻管を全剔出しなければ全治し難い。この瘻管の全剔出は割合困難なもので，在来パラフイン注入，色付絹糸の導入等が応用されている。併し炎症を起し化膿中の瘻管を全剔出することはさらに困難である。私は東京逓信病院外科の中谷氏が粉瘤や皮様嚢腫の全剔出手術の代りとして硝酸銀加硝石（通称ラーピス）を用いているのを聞き，耳瘻管の化膿を起しているものに硝酸銀の結晶を利用することを考え，現在迄に10名の患者に用いて，その成績良好なので茲に報告致します。

Higuchi suggests the use of silver nitrate crystal in treatment aural fistula. Under or-dinary circumstances the tissues surrounding the fistulous tract is naturally brittle from inflammatory involvements rendering surgical exterpation of that tract a difficulty.However, if such atract is treated by insertion of a peice of crytaline silver nitrate the size about 1/3 of the grain of rice which may left in situ for 24 hours, its dissection becomes highly facilitated with easy removal. In 8 cases of aural fistuala of the total 10, the present me-thod was succesetully employed with cure to patients within the period of 1 week postope-ratively; the method is devoid of any compli-cation and of any hemorrhage.
