瞥見麻痺の治験例

　瞥見麻痺は多発性硬化症，孤立結核結節等の腦幹疾患に於て時折認められる一眼症状であり，耳科医を最初に訪れる事は少ない．腦疾患の局所診断に対して瞥見麻痺は屡々決定的な意義を有するが，そし麻痺が核上，核末梢性であるかの区別は，耳科学的検査．即ち迷路性眼球反射検査にかゝつている．その簡単な方法として用いられるRoth-Bielschowsky氏法も，1/2-/4回転による迷路刺戟が眼球に及ぼす反射を検する方法である．

　飜つて，耳科医にとつても，瞥見麻痺の理解は迷路性眼球反射，特に眼震の中枢機制を理解し，又更に中枢迷路及び前庭神経中枢導路の研究に貢献する点大である．瞥見麻痺に於ける迷路性眼球反射について記載したのは，Wernicke氏仮性眼筋麻痺に於けるOppenheim，Roth，Bielschowsky等の記載をもつて初めとし，その後多数の報告を認めるが，瞥見麻痺に於ける前庭機能を詳細に，且つ系統的に記載したのは，Baranyを初めとする．

　ROKUGO reports a cass of paralysis of the mechanism by which conjugate occular movements are maintained with observations on the progress to a cure. In the beginning of the treatment no response whatever could be induced by vestibular stimulations but, gradually, there was a change whereby, when cold stimulation was given the left ear, the cold albne was capable of restoring that movement which increased in degree as long as the stimulation was continued. Following this procedure the conjugate occular movement was recovered but still, the vestibular nystagmus could not be elicitted.
