耳前よりする中耳手衞—Otto Popperのtranstympanic approrchの紹介

　中耳の手術はSchwartzeのAntrotomie以來既に半世紀以上も耳後より行はれることを原則として來た．それは炎症の主要部位をなす蜂巣群が主として後方に拡つてゐることに基因してゐるが單にAttic或は鼓室腔内のみの処置を問題とするときは必ずしも最適の進路ではない．蜂窠発達の著しく抑制せられたものでは空腔は單にAttic及びAntrumに限られてゐるから後方から開放するためには非常に厚い象牙質樣の骨壁の除去を経なければならぬし又進路が斜になることからAtticのOrientierungも必ずしも容易ではない．Stacke及びZaufalの術式は之のSchwartzeの術式の不利から免れるために直接Attic外壁及外耳道上後壁の鑿除より手術をはじめて直接Attic及Antrumの開放を企てたものである．然し之等の術式も同樣に進路を耳後Postauralにとつた．

　耳後から進むことに対して直接耳内endauralに外耳道内より手術する術式は既に1912年Kashiwabara及びThies父子によつて提唱せられてゐる．之の術式はAttic及びAntrumの開放に対してその手技の極めて保存的であり局所解剖学的な位置判定の容易な点から爾來種々なる改変が加へられて尚特種なる位置を保つて來た．例えばEicken-Thiesのendaurale AntrotomieとしてSchwartze-Zaufalのpostaurale Antrotomieに対して來た．1929年米のLempertは外耳道内三角形の皮膚瓣切除を行い電氣ドリルを用ひる方法をSubcortikale Antrotomieとして発表した．1934年Matisは皮膚切開をCavum conchaeに加へことにより廣い術野を得，之をConchale Ra. Op. として報告してゐる．Lempertの術式は其の後皮膚切開にも改変を加へ現在迷路Fenestrationの進路として又endaural mastoid operationとして脚光を浴びてゐるととは衆知の事である．（附図参考）

　Goto says in supporting Popper' s transtympanic approach in otitis media operations that classical postaural route for attico-antrostomy or radical mastoidectomy on sclerotic mastoid bones, in which the affection is limited to the attic and antrum, is now outmoded because it requires removal of thick bony wall, which is shock provoking to the patient, before attaining the object, More over to a novice orientation of difficulty and this structure is exposed to chances of injury. Transtympanic approach for fenestratfon operation employed by Popper is a new route wich precludes precarious obstacles met in others. With availibility of direct view in the operative field at a shorter distance from the poit of entry it is a method of choice which stands over and above endaural method that has been advanced by Eiken-Theis, Matis and, Lempert.
