オドワイヤー氏挿管用具に対する一考案

　ある種の急性上氣道性呼吸困難の救急処置として又慢性器質的喉頭狹窄の拡張法として用いられるオドワイヤー氏挿管法に必要な所要用具はオドワイヤー氏創案以來諸家によつて幾多の改変が加えられて今日に至つておるが，私は最近一喉頭狹窄患者に於て本法を應用するに当り若干不都合な点に遭遇し，之に少しく改変を加えたので茲に私のその改良型について申述べ，読者諸賢の御批判を仰ぎたいと思う．

　扨て本法を支障なく実施するには相当の熟練を必要とすることは一度本法を行つたことがある人なら誰しも経驗するところであろうが，私が本法実施に際し不都合と感じた所が以下記載の如く三，四点みられた事は私の未熟に由るものと考えられないでもない．しかしそれはともかくとして私が不都合と思つた諸点を次に述べてみたい．

Nishibe says laryngeal intubation by Odwyer's method is not without difficulties in that, (1) the directing finger may not be long enough to reach over the epiglottis especially in adult ca-ses, (2) the technique requires much practice, (3) the tube may be easily displaced at the time of the removal of obturator, and, (4) the tude may be also easily displaced by vomiting which is readily incited by withdrawal movements of the inserter. In order to circumvent these diffi-culties the author devises intubation under direct view with the aid of laryngoscope and employs an introducer the flexible end of which is slightly lengthened. Thus, the tube is stabili-zed after insertion and at the time of removal of the obturator.
