Ⅰ．はじめに

　Neurofibromatosis type I（NF−1）は第17染色体に異常のある常染色体優性遺伝性疾患であり，café—au-lait spotや多発性のneurofibromaなどの外胚葉系の異常のほか，稀に血管病変など中胚葉系の異常も合併することが知られている^1,4,10）．脳血管系に関してはこれまで閉塞性病変，動脈瘤，動静脈瘻（arteriovenous fistula：AVF）の合併例が報告されている^1-6,8-12）．

　今回，われわれは後頭部皮下の拍動性腫瘤を主訴とする頭蓋外椎骨動脈，外頸動脈のAVFを合併した神経線維腫症に対して，コイルと液体塞栓材(Eudragid)による血管内手術と外科的手術の併用が有用であった症例を経験したので，文献的考察を加え報告する.

We presented a case of neurofibromatosis type I (NF-1) associated with an extracranial arteriovenous fistula (AVF) fed by the occipital and vertebral artery. A 20-year-old man was referred to our hospital because of an occipital subcutaneous pulsatile mass. A CT scan showed a huge subcutaneous enhanced mass. Angiography revealed that the occipital AVF was fed by bilateral occipital arteries, the left ascending pharyngeal artery, the left middle cerebral artery, and the left vertebral artery withabundant communication with the subcutaneous veins.
