I．はじめに

　内頸動脈の一側もしくは両側欠損は比較的稀であるが，現在までに一側欠損53例，両側欠損11例の報告がある．これらの中に脳動脈瘤の合併は一側欠損では19例，両側欠損では3例があり，一側欠損19例中11例は前交通動脈に，両側例では中大脳動脈の細菌性動脈瘤例を除くと，2例とも脳底動脈先端部に発生している．著者等は67歳の女性がクモ膜下出血で発症し，脳血管撮影と剖検で左内頸動脈欠損と前交通動脈瘤の合併を確認し得た．この一側内頸動脈欠損と脳動脈瘤の合併例について文献的考察を加え報告する．

A 67-year-old female was admitted to our hospital because of unconsciousness after severe headache on April 7, 1986. On admission, the patient was semicom-atous with anisocoria and absence of light reflex. Com-puted tomography showed subarachnoidal hemorrhage with intraventricular clots.

Right carotid angiogram revealed that the left anter-ior cerebral artery was filled from the right side through the anterior communicating artery and a saccu-lar aneurysm in anterior communicating artery. On left-side carotid angiogram, neither the left internal carotid artery nor the intracranial arteries were visualized.
