The internal carotid artery is one of the most stable arteries and it's absence is very rate. The authors reported a case of a 12 year-ole-girl who suffered from a subarachnoid hemorrhage and who found to have an aneurysm arising from the anterior communicating artery and absence of the left internal carotid artery.

This patient was admitted to Department of Neurological Surgery, Sapporo Medical College and Hospital, with the object of detail examination for her subarachnoid hemorrhage experienced one and half month before admission.

On admission, no neurological abnormality was observed.
