I.はじめに
近年isolated fourth ventricleに関する報告は増加しつつあるが,その多くは短絡術後数カ月から数年という長期経過した小児例であり,成人例での報告は著者らが探し得たかぎりではDon Defeoら1)の1例,Foltzら3)の1例と大槻ら7)の1例にすぎず,その発生は極めて稀なものと思われる.
今回われわれは57歳の破裂脳動脈瘤の1例において,約40日という比較的短期間に第4脳室が嚢状に拡大し,突然呼吸停止をきたしたが,第4脳室開放術により救命しえた本症の1例を経験したので,その経過を報告するとともに成因に関し若干の考察を加えたい.
We present a case of isolated fourth ventricle which developed after massive ventricular homorrhage, due to a ruptured cerebral aneurysm.
In an adult an isolated fourth ventricle seems to be very rare, also the sympton of unconsciousness and respiratory arrest and the rapid development in this case seemed very unusual. The aqueductal oc-clusion was considered to be related to the intra-ventricular hematoma which remained as clots over a 2-month-period of time.
This case was a 57-year-old woman admitted to our hospital in a state of coma.
