A Case of Prolactinoma in Close Association with Rathke's Cleft Cyst

I．はじめに

　Rathke's cleft cystは，嚢胞壁が盃細胞を混じえた繊毛円柱上皮，あるいは重層扁平上皮から成ることはよく知られている．Kepes^7）は下垂体腺腫内にこのようなRathke's cleft cystが存在する例について，下垂体tran—sitional cell tumorの概念を提出した．今回われわれはprolactinoma内に大きなRathke's cleft cystを伴っていた症例を経験し，腺腫細胞の中のフィラメント構造物の性状を免疫組織化学的に検討した結果，Kepesのいうtransitional cell tumorの概念に多少の疑義を感じたので報告する．

We report a case of Rathke's cleft cyst in pituitary tumor. A 31-year-old woman admitted to our hospital complained of visual disturbance, headache, amenor-rhea and galactorrhea. Serum prolactin level was 3,060 ng/ml. By means of CT scan and MRI, we found sup-rasellar-extending tumor to have cystic component in the center of the tumor. Transsphenoidal surgery re-vealed grayish jelly-like content in the cyst.

Histologically tumor cells were composed of chro-mophobic, basophilic and eosinophilic cells.
