I．はじめに

　内頸動脈後交通動脈分岐部動脈瘤や脳底動脈動脈瘤による動眼神経麻痺は，その解剖学的特徴からくも膜下出血の有無にかかわらず，しばしば出現するが^2,3,15,20），中大脳動脈動脈瘤による動眼神経麻痺は稀である．

　また，瞳孔障害がなく，外眼筋運動制限のみを呈する動眼神経麻痺"pupil-sparing oculomotor palsy"は，糖尿病性動眼神経麻痺の特徴とされ^28），動脈瘤を原因とすることはごく稀である^24,28）．われわれは最近pupil-spa—ring oculomotor palsyで発症した非破裂前側頭動脈動脈瘤の1例を経験したので，若干の文献的考察を加えてここに報告する．

The authors report a case of unruptured anterior temporal artery aneurysm showing pupil-sparing oculomotor palsy.

The patient was a 55-year-old male with the complaints of left blepharoptosis and diplopia. He had a history of tuberculous meningitis 9 years pre-viously, and since then he suffered from paraparesis of lower extremities and bladder and bowel dis-turbance. Neurological examination on admission revealed left blepharoptosis and disturbance of medial and vertical movement of the left eye ball, but the pupils were isocoric with normal light reaction.
