視力障害と動眼神経麻痺を呈した中頭蓋窩神経鞘腫の1例

Middle Fossa Neurinoma with Proptosis and Oculomotor Palsy

I．はじめに

　頭蓋内神経鞘腫は全頭蓋内腫瘍の約8％を占めるといわれている^16）．von Recklinghausen病を合併しない頭蓋内神経鞘腫の発生部位は聴神経に圧倒的に多く，稀に三叉神経にも見られる．その他，動眼神経^18），滑車神経^3,17），顔面神経^11），舌咽・迷走・副神経^7,19），舌下神経^20）が報告されているが，これらは極めて少ない上今回われわれは，眼球突出と動眼神経麻痺を呈した中頭蓋窩の神経鞘腫を経験したので，その発生部位に関して文献的考察を加えて報告する．

Proptosis and oculmotor palsy which occur with intracranial neurinoma are very rare.

Case: A 29-year-old right-handed female was admitted to the Department of Neurosurgery, Hokkaido University School of Medicine on Sept. 6, 1975. Eight months before admission, the patient complained of double vision and one month later, noticed decreasing of right visual acuity. Four months before admission, right-sided ptosis occured and she noticed right temporal visual field's defect. Two weeks before admission she became aware of proptosis of right side.

On admission, the general examination and vital signs were normal.
