I．はじめに

　家族性に脳動脈瘤が発生することは比較的稀である^3,7,12）．しかし，脳動脈瘤の家族発生は嚢状脳動脈瘤の発生原因として，先天的および何らかの遺伝的要因の関与を示唆する事実として以前から興味を持たれてきた^4）．そして，家族性脳動脈瘤ではその発生部位や発生年齢などにおいて，一般の脳動脈瘤とはかなり異なる特徴を有することが指摘されている^1,3,7,12）．さらに，家族性脳動脈瘤であっても，兄弟，姉妹例の場合にはさらに特異な特徴があることも知られている^1）．

　著者らは1982年，比較的稀な脳血管奇形を合併した脳動脈瘤を有する1姉妹例につき報告した^13）．その後さらに1姉妹，1母子例の計4例の家族性脳動脈瘤を経験した．そこで今回はこれら6症例をまとめ報告するとともに，とくに兄弟，姉妹など同胞例の特徴を中心に若干の文献的考察を行ったので報告する．

The authors describe two pairs of siblings and a mother-son with cerebral aneurysms and the charac-teristics of familial intracranial aneurysms are briefly discussed.

Family 1 : A 54-year-old hypertensive woman (case 1) developed subarachnoid homorrhage and a saccular aneurysm at the proximal portion of the left anterior cerebral artery was demonstrated on the angiogram. An azygos anterior cerebral artery was found as anassociated anomaly. A 53-year-old hypertensive woman (case 2), a younger sister of case 1. suffered from subarachnoid hemorrhage and a saccular aneu-rysm at the distal portion of the right anterior cere-bral artery was found on the angiogram. Vertebral angiogram showed bilateral fenestration of the ex-tracranial vertebral arteries as a coincidental anomalies. Neck clipping for the aneurysms were successfully done in these two cases.
