I．はじめに

　近年，悪性脳腫瘍に対して種々の抗癌剤を使用した多剤併用による寛解導入療法が積極的に行われるようになってきた^5）．ところが，大多数の抗癌剤は多様な副作用を有しており，なかでも骨髄抑制というdose depend-entな副作用は最も重篤であり^21），かつagranulocytosisに伴って重症感染症を引き起こし，いわゆる"chemo-therapy death"に陥る症例が多くなってきている．このような病態に対する治療法としては，従来より大最抗生物質投与，さらに可能ならば無菌室隔離などが行われているが，実際は感染症を有するagranulocytosisの患者に対しては無効例が多い．

　そこで白血病治療の分野では，最近agranulocytosisに伴う重症感染症に対して積極的な補助療法として顆粒球輸血の重要性が指摘されている^2,8,16）．

Recent progress of the chemotherapy for malignant braintumors has made some prolongation of survival time of thepatient.

However, it is still far away from our satisfactions andstill we are even irritable about the results of brain tumortherapies in neurosurgical practice.

One of the major side effects of these chemotherapeuticagents is myelosupression which is dose-dependent.

When applying the chemotherapy to the brain tumorpatients agressively, we experience that occasionally weare having a chance to see the so-called "chemotherapy death" caused by agranulocytosis and severe infections.
