I．はじめに

　頸部頸動脈の狭窄性および潰瘍性病変に対して，いろいろの異なった手技で頸動脈内膜切除術（Carotid Endarterectomy，以下CEAと略す）が行われる．しかし従来の方法は欧米人の頸動脈を対象としたもので，本邦例の頸動脈は，①比較的小口径で，②分岐部位置が高く，③病変は線維性粥腫が多く，④同側に頭蓋内血管病変を伴うことが多いのが特徴的である^1-4）．これらの解剖学的および臨床的特徴のために，われわれのCEA手技は若干従来の方法とは異なっているが，本邦例のCEAを安全に行うためには，そのような配慮が必要であろうと考える．以下，解剖を中心にCEAの一方法としての著者らの手技を述べるが，紙数の都合で従来の方法との比較は他の文献^1,2）にゆずりたい．

Many different teachniques have been utilized forcarotidendarterectomy (CEA). The method demonstrated hereIemployed a minor but significant alterationespecially in thetechniques for dissection and the removal of theathero-matous plaque. This was to cope with the Japaneseathero-matous plaque which is, according to the author'sstudy,characterized by the less marked atheroma ends bothin thesmaller internal carotid and common carotid arterywithhigh bifurcation. In addition, the more fibrousnature ofthe plaque requires a technique to find and developtheadequate layer for the CEA.
