I．はじめに

　脳血管障害における脳梗塞の比率は近年本邦においても増加し，中でも頭蓋外頸動脈の病変によるものが増加しているといわれる^22）．われわれも頸動脈狭窄症に対して積極的に頸動脈内膜剥離術（以下CEA）を施行してきたが^28-31），日本人の食生活が西洋化するに従って，頸動脈病変を有する症例が同時に冠動脈病変を有することも多く^{15-17,21,34）}，心筋虚血がCEA直後のpostoperative mortality上昇の原因となり，また長期follow-up中の死因で最も多いのは心筋梗塞であることをわれわれは指摘してきた^30）．欧米においては頸動脈病変と冠動脈病変の合併症例に対して，同時に手術をする症例もあり，またCEA症例の予後に冠動脈病変が関連するとする報告が早くからみられる^{2,4,6-8,11-14,18-20,23-26,32,33）}．しかし，本邦ではCEA症例に対して冠動脈造影を行って，冠動脈病変の評価を行う必要性について検討した報告は少ない^21）．われわれはCEAを行う症例に対して，術前に冠動脈造影を施行し，冠動脈の狭窄病変の科度と術前に見られた危険因子との関係，冠動脈病変に対する治療とCEA術後の予後などについて検討したので報告する

We evaluated 37 cases of coronary angiography in patients undergoing carotid endarterectomy (CEA). The severity of coronary stenosis was estimated by Gesini's scoring system. There was no correlation be-tween the severity of carotid stenosis and that of coron-ary stenosis, but those patients who had a history of coronary artery disease, carotid bruit or intracranial artery stenoses presented significant severe coronary stenosis in most cases. Even in the patients who had no history of coronary artery disease (n=26), 13 patients (50%) had stenotic lesions shown by coronary angio-graphy. Eight patients required treatment for their coronary stenotic lesions:5 were treated with percutaneuos transluminal coronary angioplasty (PTCA) and 3 with coronary artery bypass grafting.

Intraoperative occlusion tests monitored by EEG and SEP showed abnormal findings in 6 CEA operations. One of these patients received PTCA before CEA, and had a good clinical course during and after the CEA procedure.

In conclusion, in patients undergoing CEA there is frequently concomitant coronary artery stenosis. We should thus assess the coronary artery lesion more pre-cisely by coronary angiography, and should carry out prophylactic treatment for these lesions.
