頸動脈内膜剥離術バイバルーン内シャントの使用経験

Clinical Application of a Biballoon Indwelling Intraluminal Shunt System for Carotid Endarterectomy

I．はじめに

　従来本邦では頸動脈の狭窄に由来する脳血管障害は少ないとされてきたが^1），近年徐々に増加しつつあるという報告もみられ^2），今後，脳外科における内膜剥離術の機会は増えることが予想される．頸動脈内膜剥離術の術式はすでに確立されたもので大きく議論となる点はないが，術中にシャントを用いるか否かは諸家によって意見の異なるところである．われわれは手術時間を懸念することなく確実な手術操作が可能であるという基本に立ち，これまで使用を原則としてきた．しかしシャント使用はかえって手術操作を困難にし，また合併症の原因にもなるとの指摘もある．そこでわれわれはこれらの欠点を除くべく新しい内シャントを試作し^3），これまでに7症例に使用，満足する結果が得られたので，症例を呈示しながらその有用性について報告する．

Clinical applications of newly developed shunt system for carotid endarterectomy were described. The shunt is a silicone tube equipping with balloons at both ends (Biballoon indwelling intraluminal shunt). The blood leak from the gap between a shunt tube and the inner wall of the carotid artery is prevented by inflating balloons after shunt insertion.

This shunt system was used in seven cases of cerebral infarction resulted from the carotid stenosis and was proved to be more convenient than other known shunts.
