動脈瘤クリップの折損に関する金属学的研究

Metallurgical Study on Heifetz Clip Failure

I．はじめに

　脳動脈瘤に対する直達手術が行われるようになって以来，既に40年余を経過している．その間初期の銀クリップをはじめとしてOlivecrona, Mayfield, Scoville，Heifetz, Yasargil，杉田，McFaddenその他，幾多の金属クリップが考案された．脳動脈瘤用クリップの改善，進歩は脳神経外科領域へのmicrosurgeryの導入と相まって，本疾患に対する治療成績の向上に多大の貢献をなしたといえよう．いまだ破裂動脈瘤急性期の手術に関しては未解決の問題を残しているとはいえ，脳動脈瘤そのものの手術術式に関しては既にほぼ確立されたといえよう．

　しかし，動脈瘤手術に用いられるクリップの問題については，問題が皆無とはいえない．特にmicrosurgeryのもとに脳動脈クリッピング手術が飛躍的に増加，確実化するとともに，使用頻度の多いクリップに何らかの物理的あるいは物理化学的要因によると思われる事故が報告されるにいたった．その端緒は著者らの報告^6）（1976年）で，その後，浅利ら^1）（1977年），Servo　＆　Puranen^12）（1977年），Quest　＆　Countee^10）（1977年），Edner^5）（1978年），Dujovny^4）（1979年），Colombo^2）（1980年）が相次いで同様の症例を報告している．

A metallurgical study on a fractured aneurysm clip (Hei-fetz) was described. The conclusion can be summarizedby the following: 1) Metallurgical data clearly indicate that the mode offailure is intergranular creacking-robably from a stress-corrosion mechanism.

2) Surgically used aneurysm clip is always in corrosiveenvironments from the metallurgical points of view. Aaprtfrom inadequate sterilization as well as mishandling, there-fore, corrosion of a metal clip is unavoidable.

3) A clip with multiple parts and made from differentalloys may have a greater tendency to corrode than a clipof the one-piece variety.
