Ⅰ．緒論

　くも膜下出血（SAH）に続発する交通性水頭症についてBagley（1928）^6,7）が光学顕微鏡を用いた動物実験および臨床例の検討を行ない，線維増殖その他のmeningealreactionを伴ったくも膜下腔の閉塞による髄液循環障害の存在を指摘して以来，くも膜の肥厚・癒着およびくも膜下腔のfibrosisなどの変化を光学顕微鏡下に観察した報告が諸家^{5,21,28,42）}によりなされてきたが，どの部位の髄液循環障害が実際に交通性水頭症を惹起するかという点に関しては現在なお必ずしも一致した見解を得るには至っていない．

　一方，透過型電子顕微鏡の導入はくも膜自体あるいはくも膜絨毛などの詳細な観察を可能とし，その結果単にそれらの形態学的追究に止まらず髄液吸収機構に関しても興味深い報告がなされているが^{2,17,24,36,38,39,45）}，くも膜下腔そのものについては対象が空間であるという特殊性もあってかむしろ見過ごされてきた感さえあった．

The scanning electron microscopy (SEM) gives inters-ting information about the changes of the subarachnoid space.

In this study, specimens from 16 patients (one during surgery and others at autopsy) were examined by SEM: 11 cases of subarachnoid hemorrhage, 2 of posterior fossa brain tumor operated on and 3 of control cases without CNS diseases.
