I．はじめに

　大後頭孔部の腫瘍は，症状の多彩さ，不明瞭さのために，他の変性疾患などと誤られて手術的治療が遅れがちである．同部の30％が良性髄外腫瘍であることから，foramen magnum syndromeを呈する患者に対しては，常に同部の腫瘍を念頭において検索すべきことが強調されてきた．われわれは，上位頸髄神経から発生し大後頭孔を通って，後頭蓋窩に進展した嚢胞性神経鞘腫を3例経験したので，臨床症状，X線学的所見を述べ，従来の報告とあわせて，診断と治療について考察する．

Three cases of cystic neurinoma arising front the upper cervical spinal nerve roots and extending to the posterior cranial fossa through the foramen magnum were reported.

Case 1 was a 52-vear-old female presenting marked bilateral papilledema, hyperactive right knee jerk, left positive Gordon reflex and trunkal ataxia. The mass was mostly cystic and arose from the left 1st cervical nerve root. It extended to the posterior fossa, occupied the cerebellomedullary cistern and displaced the left cerebellar hemisphere posteriorly and superiorly.
