大脳鎌内に進展した巨大前頭洞Pyoceleの1例

A Huge Frontal Sinus Pyocele Extending into the Cerebral Falx

I．はじめに

　前頭洞mucocele,pyoceleは，本邦では耳鼻科領域における日常診療上しばしばみられる．前頭洞後壁の破壊により，脳硬膜が前頭洞内に広く露出しているものも稀ではない．われわれは頭蓋内に大きく進展した前頭洞pyoceleの1例を経験した．

A 50-year-old male was admitted to us presenting a bulging of 5×6 cm in size on the forehead. His general condition was excellent and neurological status was normal. The mass in the forehead was soft and showed a fluctuation. On the plain skull films, a lobulated bone defect of 5×4 cm in size was noticed in the frontal hone and the frontal sinus completely disappeared. Carotid angiograms revealed posterior displacement of the supraclinoid portion of the internal carotid arteries and the proximal part of the anterior cerebral arteries, and elevation and posterior displacement of both frontopolar arteries.
