乳汁分泌を伴った男性プロラクチン産生下垂体腺腫の1例

A Case of Prolactinoma with Galactorrhea in Man

I．はじめに

　プロラクチン産生下垂体腺腫は，女性の場合は，無月経・乳汁分泌により比較的早期に発見される比率が高いのに対し，男性の場合は腺腫増大の結果，視力視野障害をきたして初めて発見されることが多い．また，下垂体腺腫の中には腫瘍内に出血し急激な経過をとることがあり，このような状態は下垂体卒中といわれている．われわれは，視力視野障害がなく，突然の頭痛・嘔吐で発症し，乳汁分泌があり，手術により腫瘍内出血を契機として発症したと考えられる男性prolactinomaの1例を経験したので報告する．

A 28-year-old man was admitted to the KyushuUniversity Hospital with an episode of severe headache.When driving a car, he suddenly developed severeheadache and this was followed by nausea and vomiting.He had been quite well except for slightly decreasedlibido before this episode. On admission, he showedgalactorrhea. The pubic and axillary hairs appearednormal and the development of the external genitalorgans seemed normal. However, the movement ofthe sperms was decreased. Ophthalmologic examina-tion was negative.
