眼窩悪性リンパ腫の1例—特に，そのCT所見と外科的治療に関して

A Case of Malignant Lymphoma in the Orbit:With special references to CT scan findings and surgical treatment

I．はじめに

　眼科領域における悪性リンパ腫としては，球結膜および眼窩悪性リンパ腫が挙げられている．前者の頻度が極めて稀であるのに対し，後者は全眼窩腫瘍の約10％を占め，それほど珍しい疾患とはいえない^14）．しかし，この悪性リンパ腫が眼窩に原発し，更に眼窩内に限局している場合，その診断は非常に困難であり，生検や腫瘍摘出後の組織学的検索により初めて確定診断がなされるものが大部分である．一方，外科的治療についても，Kronlein's operation, anterior orbitotomy, nasal orbitotomy, transcranial operation等，その腫瘍の局在と手術目的により手術方法が異なり問題となっている．更に眼窩悪性リンパ腫は全身への移行を示すものもあり，かつ近年進歩した免疫学の観点からも新たな関心を集めている．

　このたび，われわれは一側性眼球突出を主訴とした症例において，CT scanで発見した眼窩悪性リンパ腫をtranscranial approachにより摘出し，術後の放射線療法により著明な改善が得られた，原発性と思われる1例を経験したので，文献的考察を加えて報告する．

A 34-year-old male, who complained of the right sided exophthalmos, was admitted to the University Hospital of Kagoshima on July 25, 1979. Physical and neurological examinations on admission were nothing particular except for the right sided exophthalmos. The laboratory findings including blood count and analysis, serum electrolytes, serum enzymes, immunoglobulins, fibrinogen, fibrin degradatian products (EDP), bleeding and clotting time, carcinoembryonic antigen (CEA) were all within normal limits.

Plain films and tomograms of the skull and the right carotid arteriograms revealed no remarkable findings.
