Ⅰ．はじめに

　後頭蓋窩における動脈瘤の発生頻度は脳動瘤の10-22％位といわれている．中でも頻度の高いのは，脳底動脈分岐部，椎骨動脈後下小脳動脈分岐部などである．しかしそのほとんどは，基始部に発生するものであり，末梢部に発生したとする報告は少ない．著者らはCTスキャン上，第4脳室内の陳旧性の血腫と思われ，脳血等撮影で後下小脳動脈の末梢部に生じた動脈瘤を発見したが，手術によりこの部の巨大動脈瘤と判明した1例を経験したので文献的考察を加え報告する．

A case of a giant aneurysm of the distal posterior inferior cerebellar artery has been presented, in which the preoperative sympotoms were those of a space occupying lesion of the posterior fossa. CT scan revealed a round mass in the cerebellum.

The wall of this round mass was not enhanced after an intravenous administration of iodine. At operation we found a large saccular aneurysm, about 3 cm in diameter, arising from the distal portion of the left posterior inferior cerebellar artery. By needle decompression and dissection around the aneurysm, we were able to place a Heifetz clip across the neck.
