I．はじめに

　頭蓋内に発生する解離性動脈瘤は，非常に珍しい病態として症例報告の対象となってきたが^14,9,20），最近急速に報告例が増加し，また積極的な外科的治療の対象とすべきものがかなり含まれるという報告がFriedmanら^6），Shimojiら^15）によって相次いでなされ，脳神経外科領域において新たな認識をよんでいる．われわれは最近，後下小脳動脈の解離性脳動脈瘤の1例を治療する経験を得たが，これはいわゆる主幹動脈以外に発生する解離性脳動脈瘤として極めて珍しく，また，臨床上いくつかの貴重な示唆を与えるものと思われるので，以下に報告し，あわせて若干の考察を述べたい．

The authors present a case of dissecting aneurysm of the right posterior inferior cerebellar artery (PICA) in a 47-year-old female, who suffered from mild subarach-noid hemorrhage. Right vertebral angiogram showed typical "pearl and string" sign of the PICA, but we could not fully understand the condition until a surgical exposure revealed a purple sausage-like dissecting aneurysm of the PICA. The aneurysm was wrapped with muscle pieces. Postoperatively she developed Wallenberg's syndrome, but it subsided gradually.
