Ⅰ．はじめに

　成人で発症するArnold-Chiari奇形について，その晩発性についての原因は不明である．診断については，Applebyら^1）は，背臥位にてのmyelographyの有効性を述べているが，一方Nortonら^10）は，高齢者での危険性を指摘している．手術については，脊髄空洞症の合併の有無が問題とされている^1）．脊髄空洞症合併例では，それが髄液循環障害による，hydromyelia^5）ないしは，交通性脊髄空洞症^3）であるとの考えから，脊髄空洞症の症状が出現したらできるだけ早期に手術すべきであり^1,3），第1，第2頸椎々弓切除術を含む後頭蓋窩減圧術のみとするもの^1），あるいは脊髄中心管の髄液貯溜を妨げる諸方法を加えるのが良いとするものがある^3-5,7,12）．

　今回，著者らは，50歳と比較的高齢で発症し，CTスキャンにて交通性延髄空洞症の合併を診断でき，後頭蓋窩減圧術に脳室・腹腔短絡術を加える事により術後良好な成績を得たArnold-Chiari奇形2型の1例を経験したので文献的考察を加えて報告する．

A-54-year old female with Arnold-Chiari Type II malformation was reported.

She was admitted to the Department of Orthopedic Surgery of other university hospital with nuchal pain. She was diagnosed as having cervical disc lesion and underwent a cervical traction in December 1976, but the swallowing disturbance appeared and then she developed pneumonia in February 1977. The cervical traction was stopped for the pneumonia. After recovery from pneumonia the cervical traction was done again in March 1977 for her complaint of nuchal pain. At the end of June in 1977 she again developed pneumonia caused by swallowing disturbance.
