顔面痙攣，耳鳴症，三叉神経痛に対する"microvascular decompression"による治療経験

Surgical Results of the Treatment of Hemifacial Spasm, Tinnitus and Trigeminal Neuralgia by the Method of "Microvascular Decompression"

Ⅰ．はじめに

　顔面痙攣に対する今迄の治療の方法は，いずれも顔面神経が茎乳突孔より出た末梢部で，これに加える各種の"外傷"がその主なものであった．これに対し近年，顔面痙攣の原因を頭蓋内における顔面神経の血管による圧迫に認め，この圧迫より神経を減荷する方法，すなわち，いわゆる"microvascular decompression"により，顔面痙攣を治癒せしめる方法が採られるようになった．一方同様の考え方から，耳鳴症のうち，頭蓋内における聴神経殊に，cochlear nerveの血管による圧迫のため起こると考えられるものに対しても，同様の手術法が行われ，これはまた，三叉神経痛，舌咽咽頭神経痛に対しても同様である．

　われわれは，1976年より1978年にかけて，この"microvascular decompression"の手術法で顔面痙攣，耳鳴症，三叉神経痛に対する治療を行い満足すべき結果を得た。そこで，その手術手技，成績，および合併症につき報告する．

Thirty-four patients of hemifacial spasm, 5 patients of unilateral tinnitus and 4 patients of trigeminal neuralgia were treated by "microvascular decompression" and the follow-up results were evaluated. (1) The follow-up results in 31 cases of hemifacial spasm were as follows: Excellent…16, Good…7, Fair…7, and Poor…1. The evaluation of the operative results of hemifacial spasm was made according to Janetta's classification and to the degree of postoperative hearing disturbance. Reduction of hearing acuity as a complication appeared postoperatively in 8 out of 31 cases of facial spasm.
