Ⅰ．はじめに

　外傷性後頭蓋窩硬膜下水腫の報告は散見されるが，いずれも亜急性および慢性の症例が主で，急性の経過をとる症例は本邦では報告をみない様である．我々は後頭部外傷後，急性の経過をとり，重篤なる症状を呈したが，手術により救命に成功した3症例を経験したので，この疾患の存在と臨床的意義について報告する．また後頭下に骨折を有する患者に着目する事が後頭蓋窩血腫の診断，ひいては本症の発見にも結び付く密なる関係を有することを認めたので，後頭蓋窩骨折と血腫発生の面よりみた統計結果を加え報告する．

　統計を試みた1972,6.1-1973,5.31の1年間は，軽症から重症まで比較的無差別に担送されてきた頭部外傷患者を対象とした結果であり，急性期症例が多い．

Four cases of acute or subacute subdural hygromas in the posterior fossa were reported. All showed suboccipital skull fractures radiologically.

Two cases of acute subdural hygromas were encounteded during the fiscal year from 1972 to 1973 at Toritsu Toshima Hospital. In the same period 254 patients with head injuries were admitted here and 106 demonstrated skull fractures. Among these 24 exhibited fractures in the suboccipital region.
