Ⅰ．はじめに

　手術中に動脈血圧を降下させて出血量を最少限にとどめようとする試みは脳神経外科領域においても行われてきた．髄膜腫の手術に際して無血手術を目的としたarteriotomy，血液の除去・再注入などの工夫もされた．薬物使用によるものとしてはganglioplegicsによる動脈血圧の降下を期待して，1950年Enderbyによってはじめてpentamethonium iodideが外科手術に導入され，ついで翌年にEnderbyら，Guiotらによってganglioplegicsの1つであるhexamethoniumの使用が報告されている．ganglioplegicsの範疇に入る薬物としては上に述べたhexamethonium，pentamethoniumの他に，tetraethylammonium，phenactropium，trimethaphan（Arfonad）などがある．

　脳動脈瘤手術中にWiklund^29）は28例にhexamethoniumを，AndersonとMcKissock^1）は9例にArfonadを試みている．最近になりganglioplegic actionのない新しい低血圧誘発剤であるsodium nitroprussideが脳動脈瘤手術に使用されてきた^{5，19，20，26）}．われわれもArfonad使用に際しての種々の不都合さの経験より，Hamby^11）によってもすでに推奨されていたsodium nitroprussideを応用するようになった．

Ninety cases of supratentorial aneurysms and four cases of arteriovenous malformations operated on under deep hypotension induced by sodium nitroprusside (SNP) are reported and discussed.

We used this technique routinely in 90 aneurysmal cases to compete with any acute rupture during the operation. Our experience with 4 arteriovenous malformations is very recent.

The mean arterial pressure (MAP) (monitored through a pressure transducer) was lowered to 50mmHg once the dural flap has been elevated and to 30 mmHg during the aneurysmal dissection and clipping.
