脳動脈瘤破裂後の水電解質代謝異常とその発現要因に関する研究—特に直達手術前の検討

Disturbances of Water and Electrolyte Metabolism and Its Etiological Factors Following the Rupture of Cerebral Aneurysms:With special reference to preoperative patients

I．はじめに

　脳動脈瘤破裂後にADH分泌異常症候群（SIADH）^1）で代表される中枢性低Na血症や，尿崩症（DI）などの水電解質代謝異常が出現することはよく知られている^{2,4,5,9,11,13-17,21,22,25,27）}．しかしながら，特に術前例を対象として，その発現要因に関する詳細な検討を行った報告は少ない．そこで，今回は直達手術の影響が全く関与しない術前期における水電解質代謝異常の発現頻度および発現要因に関する検討を行ったので報告する．

One hundred fourteen patients with ruptured cere-bral aneurysms were reviewed in regard to the inci-dence and etiological factors of preoperative distur-bances of water and electrolyte metabolism. Patientswith inadequate salt intake, evidence of renal disease,cardiac failure or excessive diuretic therapy wereexcluded.

Twenty-five (21.9%) patients developed water andelectrolyte disturbances. Hyponatremia (less than130mEq/1) occurred in 18 (15.8%) of 114 patients.
